Painwheel had been hesitant to fight against the army from before. Maybe it was because they were still people and she didn't feel right just killing them like that. Or she didn't want to risk the possibility of losing herself to the voices that told her to kill once she got started, and therefore hurting someone else by accident, or worse.
Whatever the reason, she wasn't staying on the sidelines anymore. Those things, those... monsters were attacking the city and it's people. Even if her consciousness was overtaken by what been done to her mind, at least Painwheel would know that she had done something to help. And Maybe, she'd be able to stop herself before anything happened, or someone would be able to do it instead.
The way she fought was frankly rather disturbing. Spears and spikes literally erupting from not just her nails and joints, but every part of her body. Piercing and skewering the monstrosities as the giant metal fan attached to her back took care of any managed to survive. More spears, as her limbs were bent at impossible angles, Painwheel grunting and screaming as her body would continue to rearrange itself into place before moving to the next target.
What injuries she received only served to fuel Painwheel's rage, her relentless attacks ceasing only when there was nothing else to set her sights on. She stood in place, breathing quickly as her glowing red eyes looked around for the next kill.

Indeed, it took a monster to destroy a monster.
